The radiograph shows interproximal overhanging material. This causes periodontal inflammation with eventual bone loss. Proper therapy would involce the following:
- Removal of all amalgam and soft tooth structure.
- Placement of composite cores.
- Crown preparations and fabrication of temporary restorations.
- Final crown preparations and reline of temporary crowns.
- Impressions.
- Try in and cementation of final porcelain/gold crowns.
